Flobert Home health care helps people remain safely at home and live as independently as possible at home during recovery from a surgery, injury or illness, managing a serious or chronic disease or dealing with multiple diagnoses. And we want to give you tools and resources to help control your disease, achieve your health goals, and live life to its fullest. Based on the services approved by the payer our team comes to your home to provide health care services such as the general ADLs and Non-ADLs: personal hygiene, grooming, feeding, laundry, cleaning and many more. We work with you and your family to give you the best care you deserve.

Our Featured Services

Community Attendant Services (CAS)

Community Attendant Services is a non-technical, medically related personal care service that is available to eligible adults and children whose health problems cause them to be functionally limited in performing activities of daily living according to a practitioner's statement of medical need. Services are provided by an attendant.

Primary Home Care (PHC)

This program is a nontechnical, medically related personal care service provided to adults whose health problems cause them to be functionally limited in performing activities of daily living, according to a statement of medical need.This service also is a nonskilled, nontechnical service provided to eligible clients who are functionally limited in performing daily activities.

Community Based Alternative (CBA)

The Community Based Alternatives (CBA) program allows eligible people to receive services in their own homes, foster homes or assisted living settings. The CBA program is the last payor of services when other third-party resources, such as Medicare and Medicaid, are available.

Family Care (FC)

Family Care is a nonskilled, nontechnical attendant care service for eligible adults who are functionally limited in performing activities of daily living. Primary Home Care provider agencies have the option of providing these services. Family Care services are provided by an attendant and do not require the supervision of a registered nurse.

Personal Care Services (PCS)

Personal Care Services (PCS) is a non-technical, medically related personal care service that is available to eligible children and young adults under the age of 21 whose health problems cause them to be functionally limited in performing activities of daily living.

Private Pay

With private duty paid in-home services, some patients pay for in-home services out of their own pocket, or from a long term care insurance policy.
